Patient Information

We use an electronic medical record (EMR) system that allows information to be accessed rapidly, that can be shared with consultants involved in your care, or to transmit prescriptions to your pharmacy.

If you have had a change in your health status since your last visit (such as a hospitalization, surgery, emergency room visit or other illness), please inform us when making your appointment, so we can obtain the necessary information prior to seeing you.

Remember to bring either your medication bottles or an accurate list of all your prescriptions and supplements. If lab work is needed, you will be informed when scheduling your appointment whether or not you need to fast.

We welcome new patients. When calling to schedule a new-patient appointment, we will ask you some basic information to get to know you and prepare for your visit. New patient paperwork that needs to be completed prior to your scheduled appointment can either be mailed to your home or downloaded here for your convenience.

Village Family Medicine is part of Spartanburg Regional Healthcare System, located in Spartanburg, South Carolina.
Village Family Medicine offers care to the whole family, from well-child exams to treatment of chronic diseases such as diabetes, hypertension, asthma, heart disease, high cholesterol, as well as the treatment of adult and adolescent acne. Village Family Medicine is experienced in the identification and treatment of attention deficit disorder for all ages. Same-day appointments are available for acute illnesses such as respiratory and urinary tract infections, stomach upsets and other urgent problems.
