Personalized Care
As a patient you receive a personalized program of care developed by a team of highly trained medical professionals including physicians, nurses and case managers. Within 72 hours of admission, your health management specialist (case manager) will discuss with you how you will receive progress reports and other important information. We encourage you and your loved ones to attend and participate in regularly scheduled patient care conferences.

Who can admit patients to SHRC?
Any licensed physician on the medical staff of SHRC can admit a patient. Physicians interested in having staff privileges may apply for review by the medical staff team. If a physician does not wish to attend but does desire to send a patient for care, a physician can be assigned to the patient. Upon discharge the patient may return to the care of the referring physician.
