This one day program will provide a comprehensive review of the INTERACT (Interventions to Reduce Acute Care Transfers) Quality Improvement program. The program will focus on how working collaboratively with Healthcare Providers in Acute Care, Long Term Care and Home Care settings can improve the quality of care and prevent readmissions to the acute care setting. The INTERACT Program focuses on the early identification and evaluation of changes in patient condition, use of effective communication tools, and quality process improvement for Long Term Care.
The goal of this program is to provide healthcare personnel and other interested healthcare professionals with the base knowledge and tools to assist them in providing quality care related to long term care clients.
