24 practices in the Medical Group of the Carolinas have received recognition from the National Committee for Quality Assurance (NCQA) as
Physician Practice Connections®-Patient Centered Medical HomesTM (PPC-PCMH).
All 24 practices received Level III recognition, the highest level of accreditation possible. This recognition shows a commitment to quality and excellence in health care.
With this designation, The Medical Group of the Carolinas now has the largest number of PPC-PCMH-recognized physicians in South Carolina. The Medical Group of the Carolinas has practices in five counties across Upstate South Carolina and the North Carolina foothills.
PPC-PCMH practices focus on health care that forms partnerships between patients and their medical team, working together to meet the acute care, chronic disease and wellness needs of patients of all ages. These practices assist patients by coordinating care from every aspect of healthcare, from rehabilitation and nutrition, to home health and hospice. Patient Centered Medical Homes guide patients through the increasingly complex world of health care.
The Level III PPC-PCMH model provides a pathway to excellence, allowing practices to continuously monitor and improve care.
