Specimen Processing Instructions

Plasma:  Centrifuge at 3380 rpm’s (±100) for a minimum of 10 minutes

Serum:  Allow to clot.  Centrifuge at 3380 rpm’s (±100) for a minimum of 10 minutes

Centrifuge within 2 hours of collection.

Transport Requirements

Room temperature:  8 hours   Refrigerated: 5 days 

Rejection Criteria

•  If not centrifuged within 2 hours of collection.
•  Gross Hemolysis

Reference Range

Creatinine = 0.47 – 1.35 mg/dl

eGFR interpretation:

30-59 = Moderate decrease in eGFR (Estimated Glomerular Filtration Rate)

15-29 = Severe decrease in eGFR

       <15    = Kidney failure

 

Modified Cockcroft Gault CrCl interpretation:

Male:  90-139 mL/min

Female:  80-125 mL/min

Calculated eGFR uses the MDRD (Modification of Diet in Renal Disease) equation.

  (Calculation = eGFR (ml/min/1.73m2) = 175 x (Standardized SCr)-1.154 x (age)-0.203 x (0.742 if female) x (1.210 if African American).

  • No calculation result will be reported over 60.  (Will be reported as >60.) 

  • An eGFR calculation is not valid for patients <19 years old and no calculation will be reported.

  • Two calculations will print.  One for African American and one listed as other for all other races.

  • Do not use the eGFR calculation for medication dosage.

Modified Cockcroft Gault CrCl will only be calculated if the patient has a height entered in Horizon Clinicals (the inpatient EMR) and if the patient is >= 18 years old.  Also, if the patient is greater than 64 years old and the serum creatinine is less than 1 mg/dl, the calculation uses 1 for the serum creatinine.  The Modified Cockcroft Gault CrCl will be calculated with any serum creatinine – ordered as Creatinine, BMP, CMP or Renal panel.
