Collection Instructions
All specimens must be accompanied by a patient history form. In-house specimens are sent ASAP to the Surgical Pathology Laboratory fresh, unfixed, on saline-moistened gauze. Please phone histology at 560-6450 that the specimen is coming. Lab must be notified 24 hours in advance of collection. Attending physician must complete patient history form. Do not collect on Friday, Saturday, Sunday, holidays or day before a holiday, due to specimen transport requirements.

All specimens must be accompanied with a requisition form.
