Medical records are confidential and available only to people authorized to access them. You can access your medical records online through MyChart, a secure online patient portal. MyChart is password protected and delivered via an encrypted connection, so all information is safe from unauthorized access.

To obtain or transfer detailed medical records to another practice, please contact the Medical Records Office at 864-560-6273 or follow the steps, below:

  1. Download the Spartanburg Regional Healthcare System Authorization to Release and Disclose Patient Information form
  2. Print the form, fill it out and sign at the bottom
  3. Mail the form, along with a photocopy image of a valid state- or federal-issued photo ID, to one of the addresses below, depending on where you received treatment
