Obtaining Your Medical Records
Our medical records locations are currently closed to the public. However, we will continue to provide services for release of health information. Please mail a completed authorization form to the medical center where you received treatment or fax to 864-560-6073. A photocopy image or a valid state- or federal-issued photo ID must be included.
Medical records are confidential and available only to people authorized to access them. You can access your medical records online through MyChart, a secure online patient portal. MyChart is password protected and delivered via an encrypted connection, so all information is safe from unauthorized access.
To obtain or transfer detailed medical records to another practice, please contact the Medical Records offices.

- Download the Spartanburg Regional Healthcare System Authorization to Release and Disclose Patient Information form
- Print the form, fill it out and sign at the bottom
- Mail the form, along with a photocopy image of a valid state- or federal-issued photo ID, to one of the addresses below, depending on where you received treatment
