MyChart Proxy Access

What type of access do you need?

• I need access to my own account.

  Please visit our main MyChart Info page if you need help getting started or gaining access to your own account.

• I need to access my child's account.

  If this request is for a minor, please note you will no longer have access to this interactive health record once the dependent reaches the age of 18. To request a paper copy of your child’s health record, visit our Medical Records page for more information.

  To gain access to your child's account:

  • Step 1: You must be 18 years of age or older and have an activated Spartanburg Regional MyChart account. Visit our MyChart Info page if you need help getting started.
  • Step 2: Complete the Proxy Request Form by downloading it below.
  • Step 3: Submit the form one of three ways:
    • In person:  Return form (with documentation, if applicable) to the appropriate physician's office
    • Fax:  Fax form (with documentation, if applicable) to (864) 560-9112
    • Email: Scan and email form (with documentation, if applicable) to [email protected]
    • Mail: Return form (with documentation, if applicable) to:

  NOTE: Signature on form must be done with a pen. Virtual/digital signatures will not be accepted.

• I want access to another adult's account.

  Competent Adults

  A competent patient is defined as one who has the mental capacity to consent to medical treatment, including understanding and retaining information relevant to the decision, weighing that information to make the decision, and communicating that decision. You cannot initiate a proxy request for another competent adult. The patient must invite you to their own account. Adults can invite other adults to have proxy access to their MyChart account.

  The quickest and most convenient way to grant or gain access between adults is through a MyChart invitation. Each adult must have their own activated Spartanburg Regional MyChart account. Visit our MyChart Info page if you need help getting started.

  If you would prefer a paper form, please note that you must complete the Proxy Invite Form and return it in person to your physician’s office. The patient wanting to grant access to their account must be present.

  Power of Attorney / Caregiver

  If you have power of attorney or the patient is incapacitated or incompetent, access can be requested using the Proxy Request Form. To gain access to the patient's account:

  • Step 1: You must be 18 years of age or older and have an activated Spartanburg Regional MyChart account. Visit our MyChart Info page if you need help getting started.
  • Step 2: Complete the Proxy Request Form by downloading it below.
  • Step 3: Submit the form one of four ways:
    • In person:  Return form (with documentation) to the appropriate physician's office
    • Fax:  Fax form (with documentation) to (864) 560-9112
    • Email: Scan and email form (with documentation) to [email protected]

• I want to invite another adult to access my account.

  Adults can invite other adults to have proxy access to their MyChart account. No one else can initiate this process for your account; you must be the one to invite someone.

  To grant someone access to your account:

  • Step 1: The individual receiving access must be 18 years of age or older and have their own activated Spartanburg Regional MyChart account. Visit our MyChart Info page if you need help getting started.
  • Step 2: Complete the Proxy Invite Form digitally in MyChart (this is the fastest method) or by downloading the form below.
  • Step 3: If you downloaded the form, you can only submit it in person at your physician's office. The patient must be present.

  NOTE: Signature on form must be done with a pen. Virtual/digital signatures will not be accepted.

• I want to grant another adult access to my child's account.

  Parents can invite other adults to have access to their children's MyChart record. This could include a birth parent granting access to a child's stepparent or grandparent to make appointment scheduling and follow-up easier. The birth or adoptive parent or legal guardian must be the one to initiate this invitation.
