Proxy access allows you to use the MyChart patient portal on behalf of another person. Common examples of when proxy access is beneficial include using MyChart for a dependent child, aging parent or spouse.
In order to request proxy access, you must have a MyChart account. Please activate your MyChart account prior to requesting proxy access.
You can request proxy access by filling out the appropriate form(s) and:
- Returning it to the appropriate physician's office, OR
- Faxing it to (864) 560-9112, OR
- Scanning and emailing it to [email protected]

- Form B: Proxy access for a parent or court-appointed guardian of a minor child
- Formulario B: Formulario de autorizacion del padre, la madre o el tutor designado por un tribunal para acceder como apoderado en MyChart
- The parent or guardian of the minor child must fill out Form B and return it to the physician office where the child is a patient.
- Form C: Patient request to designate proxy access to another adult
- Formulario C: Formulario del paciente para autorizar el acceso como apoderado en MyChart
- The patient requesting to grant proxy access to another adult must fill out Form C and return it to the physician office where the requestor is a patient
